Midlife brings changes you might not have expected. Mood swings, anxiety, sleep problems, and feeling unlike yourself can all be connected to perimenopause. If you're in your 40s or 50s and struggling with irritability, sadness, or identity questions, you're not alone. At The Motherhood Wellness Center, led by Danielle Kornacki, LMHC, PMH-C, we provide evidence-based therapy for women navigating perimenopause and midlife mental health challenges.
Mental Health Support for Midlife Changes in Greater Boston
Perimenopause is the transition before menopause, often starting in your 40s, though it can begin earlier. Hormone fluctuations affect mood, sleep, energy, and overall well-being.
You may feel anxious, irritable, or unlike yourself. These changes are real, driven by shifts in estrogen and progesterone that impact the brain. Midlife adds further stress with caregiving, career demands, and identity questions. At The Motherhood Wellness Center, we understand how hormonal and life stage factors intersect.
We provide practical, evidence-based therapy for women across Massachusetts, including Boston, Worcester, Cambridge, and Newton, through secure, HIPAA-compliant telehealth.
Did You Know?
According to the U.S. Office on Women's Health, perimenopause can last two to eight years before menopause. Women who have a history of depression, anxiety, or postpartum mood disorders are at higher risk for significant mood changes during this transition.
Mood Swings, Anxiety & Depression in Your 40s and 50s
Mood changes during perimenopause can feel sudden and intense. You might feel fine one moment and overwhelmed or tearful the next. You might experience anxiety that seems to come out of nowhere, including panic attacks, racing thoughts, or constant worry.
Depression can show up as low mood, loss of interest in things you used to enjoy, fatigue, or feelings of hopelessness. These symptoms can fluctuate with your cycle, intensify during certain weeks, or feel constant and unrelenting.
Sleep Issues & Irritability
Sleep problems are one of the most common perimenopause symptoms. Night sweats, racing thoughts, and changes in sleep architecture make it harder to fall asleep and stay asleep. Poor sleep worsens mood, increases irritability, and reduces your capacity to handle stress.
You might snap at people you love, feel short-tempered over small things, or simply feel exhausted all the time. Irritability during perimenopause is often hormonal, but it's made worse by sleep deprivation and the stress of managing everything else in your life.
Identity Shifts: Career, Family & Caregiving Roles
Midlife brings questions about who you are and what you want for the next chapter of your life. Your children might be leaving home, creating an empty nest transition.
Your career might feel stagnant or misaligned with your values. You might be caring for aging parents while still supporting your own family, a situation known as the sandwich generation.
These role shifts can trigger feelings of loss, confusion about purpose, or anxiety about the future. Therapy helps you process these transitions and make intentional choices about how you want to spend your time and energy.
What Happens During Therapy
Therapy for perimenopause and midlife mental health is practical, evidence-based, and focused on helping you feel like yourself again. Sessions are typically 50 minutes and take place virtually via a secure, HIPAA-compliant platform. You can join from home, your office, or a private space during a break. Most clients start weekly for the first month to build skills and create momentum, then shift to biweekly as symptoms stabilize.
Evidence-Based Tools (CBT, ACT, Stress Reduction)
We use cognitive behavioral therapy (CBT) and acceptance and commitment therapy (ACT) to address the thought patterns and behaviors that worsen mood and anxiety symptoms.
CBT helps you identify and challenge catastrophic thinking, perfectionism, or negative self-talk that fuel distress. ACT teaches you how to make space for difficult emotions without letting them control your choices.
We also teach stress reduction techniques including breathwork, progressive muscle relaxation, and mindfulness practices that calm your nervous system and improve your ability to regulate emotions during hormonal fluctuations.
Values-Based Planning for Work, Family & Self
Midlife is an opportunity to realign your life with what actually matters to you. We use values clarification exercises to help you identify what brings meaning and satisfaction.
This might include exploring career changes, setting boundaries with family demands, creating space for hobbies or interests you've neglected, or redefining relationships that no longer serve you.
Values-based planning helps you make intentional choices about how you spend your time, energy, and resources during this transition.
Collaborating with Medical Providers
With your written consent, we can coordinate care with your primary care provider, OB-GYN, or psychiatrist. This might include sharing treatment goals, discussing how therapy and medication work together, or aligning on a comprehensive approach to your symptoms. Therapy complements medical care but doesn't replace it. Many women benefit from both hormone replacement therapy (HRT) and counseling, as they address different aspects of perimenopause symptoms.
